Osteomyelitis of the base of the skull secondary to Aspergillus.

چکیده
Malignant external otitis is a well-characterized entity, known for its insidious and morbid course in immunocompromized patients. Malignant external otitis occurs most commonly in elderly diabetics and is almost uniformly caused by Pseudomonas aeruginosa. If not recognized early in the immunocompromized patient, it can lead to extensive skull base osteomyelitis (SBO), with multiple cranial nerve neuropathies and eventual death. In patients presenting with multiple cranial nerve deficits, mortality ranges from 28% to 6O%.l Fungal SBO is rare, and the diagnosis can be delayed because cultures growing fungal pathogens are often interpreted incorrectly as an opportunistic colonization. This may lead to delayed treatment and greater morbidity for the patient. We present a patient with SBO secondary to Aspergillus and review the literature. His presentation was identical to that of pseudomonal necrotizing external otitis and SBO. However, his poor response to antipseudomonal therapy led to an eventual isolation and treatment of invasive aspergillosis.

Skull-base Osteomyelitis: a Dreaded Complication after Trivial Fall and Inadequate Management
Introduction: Skull-based osteomyelitis is bony infection which generally originates from inadequately treated chronic infection, adjoining tissue infection or after trauma.Case: 11 month female child had a trivial fall while standing near a bucket. The child developed fracture of right clavicle and left orbital swelling which was inadequately treated. This resulted in in spread of infection to...
متن کاملTh17 cytokine deficiency in patients with Aspergillus skull base osteomyelitis
BACKGROUND Fungal skull base osteomyelitis (SBO) is a severe complication of otitis externa or sinonasal infection, and is mainly caused by Aspergillus species. Here we investigate innate and adaptive immune responses in patients with Aspergillus SBO to identify defects in the immune response that could explain the susceptibility to this devastating disease. METHODS Peripheral blood mononucle...
متن کاملFungal infection of the sinus and anterior skull base
Abstract Background: Invasive fungal infection is an opportunistic infection caused commonly by mucoraccae and aspergillus. It mostly occurs in patients with underlying disease. Since it has a high mortality and morbidity rate, considering a treatment strategy seems necessary. Objective: Since there has not been a clear protocol for treating these patients, we decided to establish...
متن کاملSuccessful Treatment of Fungal Osteomyelitis with Voriconazole in a Patient with Chronic Granulomatous Disease
BACKGROUND Chronic granulomatous disease (CGD) is an immunodeficiency affecting phagocytic leukocytes. Defective respiratory burst mechanism renders the affected patients to be susceptible to catalase positive microorganisms. With the great successes in antibacterial prophylaxis and therapy, fungal infections are a persistent problem. Invasive aspergillosis is the most important cause of mortal...
متن کاملOsteomyelitis and lung abscess due to Aspergillus fumigatus in a chronic granulomatous disease patient
Background and Purpose: Chronic granulomatous disease (CGD) is an inherited disorder of the nicotinamide adenine dinucleotide phosphate (NADPH) oxidase complex. This disorder results in recurrent life-threatening bacterial and fungal infections. Aspergillus species are the most common fungal infections in these patients. Case Report: Herein, we present a case of fungal infection in a girl wi...
